SCLERODERMA, a disease with a name as complex as its nature, is often a life-altering diagnosis.
It’s a condition where the body’s own defence mechanisms turn against it, causing connective tissues to harden and thicken.
Royal Bahrain Hospital (RBH) rheumatologist and internal medicine consultant Dr Manar Majed Altawash explains: “The word ‘scleroderma’ comes from the Greek words ‘sklerosis’ meaning hardening and ‘derma’ meaning skin. However, it extends beyond the skin, impacting various organs and systems.”
This autoimmune rheumatic disease, as Dr Altawash describes, is not merely a skin condition, but a systemic one. The body’s immune response goes haywire, triggering excessive collagen production that leads to the hardening of tissues.
Diagnosis, therefore, is not a straightforward process. Dr Altawash outlines a comprehensive approach that involves a thorough medical history, physical examinations to identify tell-tale signs like skin thickening and Raynaud’s phenomenon (a condition where blood vessels in the extremities constrict in response to cold or stress), blood tests for specific antibodies, such as anti-nuclear antibodies (ANA) and specific scleroderma-related antibodies, and imaging to assess the extent of organ involvement.
In some cases, a skin biopsy may be necessary to confirm the diagnosis.
“Treatment is tailored to each individual, addressing their specific symptoms and affected organs,” Dr Altawash assures.
Medications like immunosuppressants and vasodilators, coupled with physical and occupational therapy, are often employed. Lifestyle modifications, like avoiding cold exposure, also play a significant role in managing the condition.
“In the journey of living with scleroderma, collaboration among various specialists is key. Rheumatologists, dermatologists and other experts like cardiologists and pulmonologists work together to provide comprehensive care and ensure the best possible outcomes for patients,” Dr Altawash notes.
“With the right interventions, support, and a holistic approach to management, individuals with scleroderma can lead fulfilling lives.”
